Benefits

401(k) Savings Plan
Employees may contribute from up to 15 percent of their compensation on a pretax basis to the WhereNet 401(k) plan. The maximum annual contribution, established by the Internal Revenue Service, is currently $10,000 per year.

Medical Plan Choices
Aetna HMO (California) - Employees choose to receive care from Aetna HMO providers. All care is managed by a Primary Care Physician (PCP). Specialist care when needed can be received within the HMO network upon referral by a PCP. Benefits for most services are paid at 100% after a $10 co-payment.

Aetna PPO (Most Locations) - Allows employees to choose between Aetna PPO and non-PPO providers to receive care. When PPO Providers are used, benefits are generally paid at 100% after a $10 co-payment (most cases). When non-PPO providers are used, benefits are generally paid at 70% (most cases) after the annual deductible has been satisfied ($1,500 individual/$4,500 family).

Kaiser HMO (Northern California) - Office and hospital visits at Kaiser facilities are covered at 100% after a $5 co-payment.

John Deere (Tennessee Only) - Employees choose to receive care from approved providers; most services are covered at 100% after a $15 co-payment.

Dental Plan
Mutual of Omaha (All Locations) - Employees can choose to receive care from any provider. Preventive care is covered at 100%: Basic care at approximately 80%: Major Care is covered in the second year of employment at approximately 50%, and Orthodontia is covered in the third year of employment at approximately 50% up to $1,500. The annual maximum benefit is $1,500 per person.

Vision Plan
Co-pay $10 for all examinations, $25 additional co-pay for materials (lenses and frames). However the $25 co-pay for materials shall not apply to elective contact lenses.
Examination - once every 12 mo.
Lenses - once every 24 mo.
Frame - once every 24 mo.
Service from a VSP Participant Paid-in-full. Non-participant up to 25%

Flexible Spending Accounts
Employees may use pretax dollars to pay for expenses not covered by the health plans or expenses related to dependent day care, by setting aside funds in one or both accounts.

Health Care Reimbursement Account:
Employees may set aside up to $2,500 each year. Examples of reimbursable expenses include deductibles and co-payments.

Dependent Day Care Account:
Employees may set aside up to $5,000 annually to pay for eligible expenses related to child care, care of a disabled spouse and/or disabled or aged parent. Eligible expenses include licensed nurseries, day care centers and individuals that provide dependent care in or outside the home.

Employee Contributions
Medical and dental accouns have a nominal pre-tax employee contribution.

Life Insurance
Life and AD&D Mutual of Omaha (All Locations) - Employees receive life insurance coverage based upon two time’s annual salary up to a maximum of $100,000. Accidental Death & Dismemberment (AD&D) Insurance is also available to employees in the same increments.

Paid Time Off
WhereNet provides regular employees with Personal Time Off (PTO) to be used for vacation, illness, emergency, etc. Employees begin accruing PTO time from their first day of employment. Employees will accrue PTO based on the following schedule:

Holidays
WhereNet observes ten holidays per year. The holidays include: New Year's Day, President's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Post-Thanksgiving Day, Christmas day, plus two holiday that are specified at the beginning of each year.

Short-Term Disability by MetLife
Loss of Time (LOT) benefit replaces up to 55% of weekly earnings less any State Disability and/or workers compensation for a duration of 12 weeks (maximum of $490. per week). (CA employees pay CA state disability and are covered by the state program)

Long-Term Disability by MetLife
Provides 90 days continuous disability up to 66-2/3% of current monthly earnings less any State disability, worker’s Compensation and/or any entitled Social security Benefits to a maximum of $7,500. per month. Monthly income up to 66-2/3% of pre-disability earnings to a maximum of $6,000 per month
